Thankyou for your letter and apologies for the slow response.  I was working in a medical school in Thailand and then transferring to Hong Kong where I will be until 20 January.

I do not claim to be an expert on the technology of incinerators although I try and follow the developments in that field.  Jim Middleton (“Clear The Air”) and others have maintained a useful flow of bulletins on that issue.  As a medical practitioner and public health specialist I have an interest in exposure assessment to dioxins and dioxin-like compounds and we have done some work in that field in recent years.  A complete set of the papers we have published so far on persistent organic pollutants is attached.  They are necessarily quite technical and may not be very useful to you.  However a one page executive summary has potentially helpful diagrams and text and the paper in “Environmental Health Perspectives” conveys some of our concerns about dioxin control, albeit from a 5 year old perspective.  They show where Hong Kong is in terms of its risk status with regard to dioxins and PCBs.

The dioxin level in human breast milk is the best and most relevant indicator of past exposure of a population or its subgroups.  It is relevant because young primagravidae (first pregnancy mothers) pass on their dioxin load to the infant during gestation and then “top it up” if they breast feed.  We usually get a large proportion of our dioxin load from our mothers, which may then be added to through the dioxin content of our diet.  So the trans generational effect should be seen as an important reason for contemporary civic responsibility.  Humans are particularly susceptible to environmental persistent organic pollutants because they are amplified in the food chain and lipophilic, so stored in body fat rather than being eliminated.

One point I should make emphatically is that we have made a special effort to ensure that although breast milk is a conduit for dioxin from mother to infant, this must not be regarded as a reason not to breast feed.  The evidence which is available suggests that breast milk is likely to protect against adverse effects at least to some extent.  This may appear to be an uncomfortable paradox but it is the best position to adopt for many reasons.

The World Health Organization (WHO) has run 3 global surveys so far on dioxin in breast milk.  We joined the 3rd one in 2001/02 with funding from the Conservancy Association.  This was the first time Hong Kong participated, although a paediatric research group (Dr Henrietta Ip) had done studies in the late 1970s and 1980s.  Comparisons between studies to identify trends are hazardous because the assays were done in different laboratories.  (Our data were generated in the WHO reference laboratory in Freiburg, the only lab with the credentials to do this in the global survey).  However it would be fairly safe to say that over the years the levels of most pollutants had declined in Hong Kong mothers possibly due in part to the closure of old incinerators as well as other regional and global regulations and controls on PCBs and sources of dioxins.

A new survey has just been completed by my colleague Professor TW Wong at Chinese University School of Public Health.  I am copying this to him so that he might add to my comments and perhaps summarize the findings of this latest data.  We have jointly applied for funding to follow-up the infants born to our 2001 mothers, but no decision yet.

My summary comments in an effort to address your questions would be:

1.   Hong Kong (and any jurisdiction) should adopt a rigorous precautionary approach to the control of pollutant emissions which bioaccumulate in animal food sources and human tissues.

2.   Dioxin compounds are rated as some of the most highly toxic chemicals in the environment.  They have been shown to be associated with severe damage to the central nervous system, immune system, reproductive and endocrine systems, skin disorders and a cause of cancers.  They are particularly important as “endocrine disruptors” in newborns and infants.

3.   Hong Kong was included in the 3rd WHO global and ranked 14th out of 26th for concentrations [Toxic Equivalents (TEQ) expressed as picograms per gram of fat (pg/g fat)] for polychlorinated benzodioxins (PCDD) and polychlorinated dibenzofurans (PCDF) (ie “Dioxins” and dioxin-like compounds).  Hong Kong ranked 17th out of 26th for concentrations of Polychlorinated Biphenyls (PCB).  The Hong Kong/PRD values were lower than at least half of the other countries in the WHO survey.

However for all of these groups of structurally related chemicals Hong Kong had the highest levels in the Asia Pacific group which included USA, New Zealand, Australia, Philippines and Fiji.  Hong Kong levels were 40% above those of Australia.

4.   The particular susceptibility of Hong Kong mothers is emphasized by the strong association between age and body levels of dioxin.  Age of mother at age of first pregnancy dominates the risk profiles in terms of higher levels.  Late age at marriage and first pregnancy in Hong Kong means that even relatively lower life-time exposures will lead to substantial bioaccumulation and higher levels of dioxins being transferred to the unborn infant.  This observation emphasizes the need for an integrated environmental and public health approach, something which the present Environment Bureau may be incapable of achieving.

5.   Emissions of persistent pollutants contaminate both land and marine based sources of food at the lower end of the food chain.

Arguments based on emissions being “blown away” by prevailing winds are scientifically unacceptable and seriously misleading if they are allowed to influence policy making.  I understand these are suggestions made by EPD/EB staff to rationalize the project on Shek Wu Chau.

6.   The lower than average (or median) detected levels of dioxins in our survey are encouraging but extreme vigilance is required to ensure than further progress is maintained.  However in our paper in Environmental Health Perspectives 2006 we wrote:

“the public health threat of dioxin contamination in this region remains a concern.  The dioxin inventory in mainland China for 2002, the first so far, estimated PCDD/PCDF emissions to the atmosphere to be 7,144-13.575g International (I)-TEQ/year (Jun et al. 2004).  The China inventory does not include uncontrolled backyard burning of waste.  In Hong Kong, the annual emission was estimated to be 23-33 I-TEQ/year (HKEPD 2000).  One main reason for the high dioxin emission in mainland China is the heavy municipal waste management burden, together with its expanding population.  Most of the municipal waste incinerators in mainland China did not comply with the PCDD/PCDF national emission standard of 1.0 ng 1-TEQ/m3, which is less stringent than 0.1 ng 1-TEQ/m3 used in developed countries (Jun et al. 2004).  The growth of the economy was associated with the development of industries causing dioxin emissions, such as crude steel and chloralkali industries.  Active dioxin abatement policies and measures are needed to enforce a more stringent PCDD/PCDF emission standard for incineration, control illegal smuggling of electronics waste (Basel Action Network 2004; Schmidt 2002) for recycling activities and prohibit uncontrolled waste burning and ensure correct disposal of equipment and material containing PCBs.”

These caveats about mainland China can and should obviously apply to Hong Kong today.  In 2006 we assumed that any incineration technology adopted in Hong Kong would be state-of-the-art and ensure that pollutant emissions were minimized to the greatest possible extent.

This now appears not to be the case and is unacceptable from a public health viewpoint.  This is especially so, given that the EPD is apparently defaulting to depending on air movement to mitigate the impact of emissions locally and will only monitor mass concentrations four times a year.

7.   From an overall project management point of view I also would want to see an EIA which takes account of the high volume of marine traffic which will be needed to transfer waste to Shek Wu Chau.  If present types of fuel oil are used to power these vessels they will add another layer to pollution of Hong Kong’s inshore waters.

8.   At a personal level, from both an ecological and aesthetic point of view I would be implacably opposed to further despoiling of the seascapes in this part of the Hong Kong archipelago.  I cannot image how an option appraisal led to this choice.

Conclusion

My strong recommendation is that the further development of incineration should be based on advanced technology, particularly high temperature systems which destroy important toxins; rigorous independent environmental impact assessment; adoption of the precautionary principle; clear pathways for ministerial and bureaucratic accountability (almost totally lacking in environmental matters at present) and failsafe continuous monitoring linked to mechanisms for totally independent oversight of this aspect of environmental management.

The Environment Bureau should recognize that there is a total loss of public confidence in its current management practice and the incinerator proposal is a new entrant in this category.

I will help if I can, though my available time is severely limited at present because of many other commitments.  Also the public health concerns are clear and unambiguous: there should be the maximum possible reduction of all combustion emissions from waste incineration and the maximum possible reduction of waste generation on the shortest possible timescale.  I am concerned that Professor Northcote Parkinson’s satirical law, “Demand expands to fill capacity”, may be the main outcome of building a so-called super incinerator.

In the meantime please see if the Executive Summary (single sheet) and the paper in Environmental Health Perspectives lead to any further queries which we might try and answer.  Note these papers are the first two attachments.

Javelin Park Investigation – UK plant filters ‘let particulates through’

STRICT regulations forbidding the emission of tiny particles from incinerators are needed to safeguard public health, warns Dr Dick van Steenis.

Ultrafine PM1 and PM2.5 particulates which are released when waste is incinerated are causing premature infant and child deaths, he believes.

Dr van Steenis also claims these particles, which are small enough to be inhaled by humans, are linked to a host of other adverse health effects, including birth defects, childhood cancers, respiratory illnesses and heart attacks.

In evidence submitted to a House of Commons select committee in May last year, Dr van Steenissaid the Environment Agency had admitted that 90 percent of PM1s and 35 per cent of PM2.5s escape through filters installed in UK incinerators.

This, the retired GP claimed, meant that UK incinerators were emitting somewhere between 40 to 120 times more particulates than those in Finland or Sweden, where air quality regulations are tighter.

In his submission to the Environmental Audit Committee, Dr van Steenis also criticised the Health Protection Agency for failing to examine data at ward level, which would allow the HPA to assess the health impact of incinerator fumes.

The HPA has so far refused to look at ONS figures despite it being recommended by the World Health Organisation, he said.

GLOSVAIN protesters and Lib Dem county councillors have been calling on Gloucestershire County Council to consider a mechanical biological treatment (MBT) plant as an alternative to the Javelin Park incinerator.

However, Dr van Steenis is equally opposed to that option on the grounds that a large amount of leftover waste from MBT plants still has to be incinerated or put into landfill.

“What we should be looking at are plasma arc gasification plants because they break everything down into atoms,” he said.

Dr van Steenis believes plasma arc gasification plants are the safest and most environmentally friendly option and are capable of generating more energy from waste than incinerators.

It says the chair of EIA subcom who works in HKPC, and HKPC has got a work contract with EPD to measure the air quality around SKC after the first round of EIA meetings for the incinerator.  So you may wonder why the chair of EIA subcom did not declare potential conflict of interest before the EIA meeting on 5 Dec and on 30 Dec (even this is the second round to review the the same EIA of incinerator).   There was another member who has declared potential conflict of interest in the EIA and ACE meetings, and she was advised to excuse herself from the meetings.

Making a hash of dash to ash

The trouble with taking on the government over its decisions is that the merits of the case get lost and it becomes an issue of face. Take the proposed monstrosity that the so-called Environmental Protection Department has planned for the scenic island of Shek Kwu Chau near Lantau. If all goes to plan, the scene of tranquility is to be transformed into a monster incinerator complete with a massive chimney stack. This site somehow received the green light in favour of an area near the landfill site near Tsuen Mun.

The other startling aspect of this scheme is the technology. The government is planning on using old moving-grate technology, which essentially moves the material over a grate and burns it. In the process it creates a noxious chemical cocktail that is either vented from the giant chimney or mixed in with powdery ash that will have to be transported by barge to a dumping ground providing further opportunities for polluting the air en route.

The various bodies involved in making decisions appear to know little about plasma arc technology. Instead of burning the waste, it is vapourised. The resulting gasses are captured instead of being vented and can be used to generate power or to create organic fuels such as ethanol or jet fuel. The solid residue can be broken up and used for making roads.

Not only is this technologically more environmentally friendly but is cheaper than the old technology. However, the government already appears to have decided against it, murmuring that the new technology is untested. But it has been used in Japan since 2002. The Tees Valley in Britain is set to build a substantial waste-to-energy plant using plasma arc technology, as is Ottawa, Canada. Other projects harnessing the technology can be found in Milwaukee, Shanghai and other cities around the world.

At the same time, a team from Imperial College London has been commissioned to carry out a survey on behalf of Britain’s Health Protection Agency after fears emerged about the health risks posed by incinerators, particularly for young children.

This survey was commissioned after alarming discoveries of the higher incidence of infant deaths among those living downwind of a number of incinerators in Britain. These incinerators are of the type being proposed for Hong Kong. Surely Asia’s world city can do better than this?

Government-appointed advisers have endorsed an Environmental Impact Assessment report on an administration plan to build an incinerator to burn Hong Kong’s municipal solid waste.

The government is considering building an incinerator at either Tuen Mun or Shek Kwu Chau, as a way to dispose of waste, because Hong Kong is running out of landfill sites.

The Chairman of the Evironmental Impact Assessment subcommittee, Tsang Kam-lam, said they gave their backing after taking all the scientific evidence into consideration.

The government is facing the threat of a judicial review if it decides to build a waste incinerator on ShekKwu Chau.

The co-owner of a restaurant on Lantau Island, Tom Hope, submitted a formal notice to the Environment Secretary, Edward Yau, and the chairman of the Advisory Committee on the Environment, Professor Lam Kwan-sing, saying that if the government goes ahead with the plan, then a judicial review will be launched.

they are suspected of causing cancer and are widely distributed throughout the food chain.

Of other air pollutants, acid gases and particulates, for instance, can harm people with respiratory

illnesses. A report from the Department of Health’s Committee on the Medical Effects of Air

Pollution (COMEAP) concluded that PM10 pollution from all urban sources hastens (‘brings

forward’) 8,100 deaths/yr, and increases or brings forward hospital admissions by 10,500/yr.”

Small area study of mortality among people living near multiple sources of air pollution

Abstract
Objective—In the area of Malagrotta, a
suburb of Rome (Italy), a large waste disposal
site, a waste incinerator plant, and
an oil refinery plant became operational
in the early 1960s and have represented
three major sources of air pollution. To
evaluate the potential health risk due to
airborne contamination around these
point sources, a small area analysis of
mortality was conducted. Cancer of the
liver, larynx, lung, kidney, lymphatic, and
haematopoietic systems were evaluated.
Methods—Sex and age specific mortality
(1987–93) and population denominators
(1991) were available for the census tracts
of the metropolitan area of Rome. Standardised
mortality ratios (SMRs) were
computed separately for males and females
in bands of increasing distance
from the plants, up to a radius of 10 km.
Stone’s test for the decline in risk with
distance was performed with increments
in radius of 1 km; SMRs were also
computed after adjusting for a four level
index of socioeconomic status.
Results—No overall excess or decline in
risk with distance was found for liver,
lung, and lymphohaematopoietic cancers
in either sex. For laryngeal cancer, an
increased but not significant risk was
found at 0–3 km and at 3–8 km. A
significant decline with distance in
mortality from laryngeal cancer was
found among men (p=0.03); the trend
remained after adjusting for the
socioeconomic index (p=0.06).
Conclusions—The study showed no association
between proximity to the industrial
sites and mortality for most of the
several conditions considered. However,
mortality from laryngeal cancer declined
with distance from the sources of pollution.
This result is interesting, as previous
findings of an increased risk of laryngeal
cancer near incinerators have been controversial.

So-called studies are often fraudulent by fiddling data, dates, areas or controls, and omitting key references. Govt.check and censor media & UK medical & scientific journals. Truth is hidden, only is spin allowed. Subsidy of BP /AMOCO Oil Co. set up FoE c.1973 to protect oil cartel etc.& to control action groups (3x£2000pa each) = £64200 of £8.1m income 2002. Patient “charities” are mostly controlled by drug co. donors and refuse to look at causes of the illness they use to collect money. Blair gave judges £1.5 million each pension fund (?for loyalty), Blair’s pal Hodge runs Legal Aid, and Solicitors have been bribed to loose cases. NHS Whistle blowers have been sacked or suspended.

Av. 12 years was taken off lifespan. Cancer incidence rose 480% on top of Belgian national increase. Slough had SMR 88 in 1990. After 5 years of Grundon (plus it appears CHP Slough & ??previous clinical waste hosp. Incinerator) SMR rose to 122 equal to 11years off lifespan by 1995 and SMR121 in 2001. This meant cull of population especially in 50’s.

Advisory council gives green light to incinerator

Residents and environmentalists opposing the construction of a super incinerator off Lantau have been dealt a blow after the government’s environmental advisory body officially endorsed the plan yesterday.

However, the endorsement has fuelled momentum for a looming legal challenge against the massive project to be built on 16 hectares of reclaimed land at Shek Kwu Chau, an island south of Lantau that for many years has housed a drug rehabilitation centre.

A retired solicitor and Lantau Island restaurant owner yesterday said he would seek a judicial review of the decision if the government continued to push ahead with their original plan to build the incinerator, which will be able to process 3,000 tonnes of rubbish a day.

The threat came as the Advisory Council on the Environment yesterday approved the multibillion-dollar project at a meeting yesterday, which means the authorities can proceed with the project if funding is approved by the Legislative Council.

As the meeting took place, about 50 opponents of the project staged a noisy protest outside.

It was the second attempt by the Environmental Protection Department to push through the project in the advisory body, after sidelining it during a court case relating to the Hong Kong-Zhuhai-Macau bridge project earlier this year.

Environmentalists and other opponents to the project say the super incinerator, as one of the largest and most expensive of its kind in the world, is not the best option for waste management and would pose a threat to marine life in the area. The project will cost between HK$8 billion and HK$13 billion.

Eddie Tse, a member of the alliance opposing the incinerator, criticised the advisory council yesterday for hastily approving the project at a closed-door meeting.

Following the endorsement, which includes an option to build the facility in Tuen Mun, the EPD will finalise the plan and table it in the Legislative Council to seek funding.

Advisory council member Tsang Kam-lam said yesterday that the environmental assessment report so far had shown that the project would not harm its surroundings. He also admitted that some members preferred Tuen Mun as a location.

Tom Hope, who manages a restaurant at Cheung Sha Beach, said yesterday he and others would launch a judicial review if the government proceeds with the incinerator on the grounds that it has not adequately explored the best technology for waste management.

Hope yesterday sent a formal notice to the Secretary for Environment Edward Yau Tang-wah, Chief Executive Donald Tsang Yam-kuen and members of the advisory council.

The Environmental Protection Department declined to comment on any legal challenge.

Australia – The Alliance for a Clean Environment (ACE) is celebrating their win in convincing the WA Government that it should not approve incinerator technologies for the Eastern Region of Perth, Western Australia.

The East Metropolitan Regional Council (EMRC) had submitted four potential incinerator technologies to the Environmental Protection Agency (EPA) as part of their proposed Resource Recovery Facility. The EMRC have now withdrawn them from the assessment process.

“This is a real success story. This regional council has been promoting incineration for 11 years, one of the longest community engagement processes I have ever been involved in!” exclaimed ACE Coordinator and Spokesperson Jane Bremmer. “Although the fight is not over yet as Kwinana and Canning Vale are also considering incineration technologies, we are confident that the EPA will also reject other proposals in WA, as it has done in the eastern region, based on the convincing, common sense arguments we have provided to them,” she added.

“We couldn’t have done it without the expertise and support of Dr. Paul Connett, an internationallyrecognised waste expert who was kindly funded to come to WA by the National Toxics Network (NTN). We are all proud participants of the International POPs Elimination Network working towards a toxic free future.”
